Please complete the Health Department form and mail it to the Orange County Department of Health.  The address is on the form.

 

All food and game vendors MUST supply a certificate of insurance naming the Otisville Lions Club, Village of Otisville and Town of Mt. Hope as additional insured.  Please mail the certificate with your payment to the Otisville Lions Club (address below). 

 

Fee:  $200.00.  Please make checks/money orders payable to:  Otisville Lions Club

Application, payment and certificate are due on or before August 1, 2012.

                                                                    BOOTH RULES & REGULATIONS

 SETUP:  Thursday & Friday 9:00 am – 2:00 pm; Saturday 7:00 am – 10:00am ONLY.  NO VECHILES MAY ENTER THE FAIRGROUNDS AFTER THESE HOURS FOR ANY REASON, THERE WILL BE NO EXCEPTIONS.  If you need to set up after these hours you must transport your merchandise into the fairgrounds by hand, hand truck, etc.  NO vehicles are allowed to be parked in the vendor spaces they must be moved to the designated vendor parking area.  Vendor booths are intended for selling merchandise only, not for parking spaces.

BREAK DOWN:  Sunday at close of Fair.  NO VEHICLES MAY ENTER THE FAIRGROUNDS UNTIL THE FAIR IS OFFICAL CLOSED ON SUNDAY EVENING, NO EXCEPTIONS.  If you need to break down earlier you must transport your merchandise off the fairgrounds by hand, hand truck, etc.  This is for insurance reasons. 

 DONATIONS:
 ñ     Food and game booths - $200
            Food and game vendors must supply a certificate of insurance naming the Otisville Lions
            Club, Village of Otisville, and Town of Mount Hope as additional insured.  This insurance         certificate must be submitted with your application.   Food and game vendor applications,   payment and certificate are due on or before August 1, 2012.  Food and game vendors cannot add new items to their booths without the written consent of the Fair Committee.  Please list all items (old and new) that are to be sold.  Food vendors may only sell 16.9 oz. bottles of  water and bottles will be sold for $1.00 each.  Reminder: Any new items listed will need to be approved by the Fair Committee.   If a food vendor is selling an item not approved by the Fair Committee they will be asked to close their booth for the remainder of the Fair.  (Food and game booth items will not be duplicated).  Food vendors have the right to sell their listed  products, exclusively, as long as the food booth operates in consecutive years.  If you do not operate your food booth one year, the product you previously sold will be offered to another     group or vendor and cannot be promised to be given back to you if you return the next year. The Fair Committee makes the decision where food and game vendors will be located during the Fair.  Application packets are due by August 1, 2012.  If your application packet is not  received by the due date your food or game booths may be offered to another group or vendor.
 

ñ     Craft and new merchandise vendors - $75 (20'x20' open space or 10'x15' under tent, electric included)  No beverages including water or food are to be sold in these spaces unless they have written permission from the Fair Committee.

 

ñ     Corner spaces - $100 (there are no corner spaces under our tent, electric is included)  No beverages including water or food are to be sold in these spaces unless they have written permission from the Fair Committee.

ñ     Garage sale/Flea market spaces - $50 (open space, you may bring your own tent)  No beverages including water or food are to be sold in these spaces unless they have written permission from the Fair Committee.

 

ñ     All vendors please make checks/money orders payable to:  OTISVILLE LIONS CLUB and mail to:  Sue Marshall, 3 Libert Court Apt. 174, Warwick, NY 10990.  Along with your application, insurance certificate (food & game vendors only) and self-addressed stamped envelope (SASE).  Confirmation and a vendor parking permits (2 per booth) will be sent to you after receipt of payment.  A SASE IS REQUIRED WITH YOUR PAYMENT AND APPLICATION.  A confirmation letter and vendor parking permits (2 per booth) will be sent to you after receipt of payment and in the SASE.  If no SASE is received please go to Lion Booth for your paperwork.

ADDITIONAL INFORMATION:.

1.      The Otisville Lions Club, Village of Otisville, Town of Mount Hope, and any property owner connected with the Fair, will NOT be responsible or liable for the damage or loss of any kind to vendors or displays, or any injury during the Fair, including setup and breakdown.

2.      The Otisville Lions Club will provide publicity for the Fair.

3.      Vendors must provide their own canopies or tents, tables, chairs, extension cords, etc.

4.      NO ALCOHOL, DRUGS, SMOKING, OR PETS (except service animals) ALLOWED ON THE FAIRGROUNDS.  The vendor or exhibitor agrees NOT to engage in any activity that detracts from the atmosphere of the Fair, including bringing pets, the consumption of alcoholic beverages, the display of pornographic materials, drugs, etc.  No silly string, poppers or bombs are allowed to be sold.  No bull horns and no loud music are allowed from any food, game, or vendor booth.  Warnings or expulsion are the penalties.  All decisions of the Lions Club are final.

5.      The vendor or exhibitor is responsible for the removal of their displays and all debris.  If debris is left at the site, the vendor or exhibitor will be billed a $75.00 clean up fee.
6.      The Fair Committee reserves the right to approve or reject all applications.  The Fair Committee's decisions are final in all matters of the Fair.

7.      Craft vendors and garage sale/flea market merchandise will be separated. 

8.      For insurance purposes and the safety of our patrons all merchandise must be kept neat and orderly. Items must be displayed on tables, if possible, and not clutter the walking space within the booth.  Vendors may not sell anything outside their booth.

9.       Failure to follow all the rules & regulations will result in not being asked to return the following year.  No monies will be refunded to any vendor who is asked to leave for breaking the rules & regulations.

10.  NO UNINSURED VEHICLES ARE PERMITTED ON THE FAIRGROUNDS AT ANY TIME.  THIS INCLUDES ALL OFF TERRAIN VEHICLES, INCLUDING ATV'S, GOLF CARTS, ETC.  PROOF OF INSURANCE WILL BE REQUIRED FOR ALL VEHICLES.
